EAISON Referral Form
The EAIs-ON program or Early Assessment and Intervention Services for Outcomes Now is a program designed to assist families with the following:
  • Prevention
  • Home-based Interventions
  • Community-based Interventions
  • Crisis Intervention
  • Counseling & Therapy Services
  • Substance Use/Abuse Interventions
  • Screening & Assessment
Referral Criteria:
​1)            At least one family member reside in Boone county and that they are able to receive services in Boone County. 
2)            There is a current risk of child/abuse and neglect (parent has to have contact with child).
3)            If the children have been removed, that the referral is made within 30 days of removal.
4)            EAISON has several components and the family needs to participate in more than one component to qualify.
5)            That the family be able to benefit from brief (90-120 days) intense, intervention.
To refer a family for services, print and complete the above referral form.
